ü  Duly filled APPLICATION FORM (Schedule-I) with all annexures.

 

 

1.    Mark Sheets & Pass Certificates:


ü  Class 10 (HSLC) or equivalent Admit Card & Pass Certificate.

ü  Class 12 (10+2 Science) or equivalent Mark Sheet & Pass Certificate.

 

 

2.    Identity & Residence Proof:


ü  Permanent Residence Certificate (PRC).

 

 

3.    Category Certificates (if applicable):


ü  Caste Certificate (SC/ST).

ü  OBC/MOBC Non-Creamy Layer (NCL) Certificate issued by the District Commissioner (DC).

ü  Economically Weaker Section (EWS) Certificate from the concerned authority.

 

 

4.    NEET-UG 2025 Documents:


ü  NEET-UG 2025 Admit Card.

ü NEET-UG 2025 Score Card

 

 

 💸 Counseling Fee Details

 

ü  Amount: Rs. 500/- (Five Hundred Rupees Only).

ü  Payment Method: Via Treasury Challan under the Head of Account “0210-03-102-0000-000-00-00-EE-V-GA”.

ü  Submission: Bring the copy of the paid challan (Original/Duplicate/Triplicate) to the counseling venue.

🔔 Special Notes for Reserved Categories

 

 

ü  For OBC/MOBC Candidates: You must submit the original Non-Creamy Layer (NCL) certificate from your District Commissioner (DC). Ensure Annexure-III of the application form clearly states your NCL status.

ü  For EWS Candidates: A valid EWS certificate from the concerned authority is mandatory.
